If you have not been referred to me by your provider, or you are a new client inquiring on availability, please do not fill out this form. Utilize the link after the contact form to see if I have availability.

Important reminders:
If you do not see any availability on my schedule (linked above), then I do not have availability. Also, I do not have days or times open outside of what is available on my schedule.
If I have seen you through the Dept. Of Corrections I cannot see you privately, regardless of if you are off supervision. This constitutes a dual relationship which is against my code of ethics.
